How to make a referral
Anyone can initiate a referral to Triumph. Our clinical liaison will complete an on-site screening to determine the patient's medical needs. Transfers often occur the same day.

Wound Management Program

Triumph Healthcare - Medically Complex Program
At Triumph, we believe the most comprehensive approach to wound management involves an interdisciplinary team of professionals collaborating to evaluate and develop a plan of care focused on promotion, maintenance, rehabilitation and restoration of patient health. Triumph Wound Management provides the community with a full-service wound management program. We serve as a referral center community physicians, providing complete wound management assessments.


Chronic Wounds 
Our patients require high acuity wound management for draining wounds and fistulae and complex management of the acute or unstable chronic wounds. Chronic wounds are classified in the following categories:

 Pressure Ulcers
 Ischemic Ulcers
 Post-Surgical or Post-Trauma
 Diabetic/Neuropathic Ulcers
 Venous Stasis Ulcers
 Peripheral Vascular Disease
