How to make a referral
Anyone can initiate a referral to Triumph. Our clinical liaison will complete an on-site screening to determine the patient's medical needs. Transfers often occur the same day.

Triumph Healthcare - Medically Complex Program
Triumph's Pulmonary Program addresses the specific needs of patients with acute pulmonary problems. Pulmonologists and Respiratory Therapists evaluate patients to create and implement individualized care plans. Weaning protocols are utilized with our ventilated patients in conjunction with other treatment modalities.
